How Scarless Breast Reduction is Performed

Dr. Lawrence N. Gray performs an all-liposuction scarless reduction, near Boston, Massachusetts, at his New England surgical facility in Portsmouth, New Hampshire. Scarless breast reduction only requires a single incision, leaving just a single, inconspicuously placed, quarter-inch scar on each breast, a drastic improvement over the old method. This tiny scar often disappears from sight over time. For the first 2-3 months, your scar will be visible, but still quite small. With massage, the scar tissue will break down and the darker color on the surface of the scar will fade over time. 

In the procedure, a surgical tube, called a cannula, is inserted into the breast through a tiny incision. Dr. Gray moves the tube back and forth, breaking up layers of fat, which the cannula removes with suction. When the fat has been removed, decreasing the size of the breast, the remaining skin will naturally shrink to fit the new breast volume, no matter how old the patient is, or how large her breasts were. Dr. Gray is able to achieve an average reduction of two cup sizes in each breast. The average volume reduced is 800 cc’s, ranging from 250 cc to 2,650 cc. The amount of fat in the female breast ranges from 61 to 74 percent. Through liposuction, Dr. Gray can remove the unwanted fat tissue while leaving the glandular tissue in place. 

After scarless breast reduction, the skin shows a dramatic degree of shrinkage: the nipple/areola is often elevated an average of six centimeters, ranging from two to 13 centimeters. This result is best seen after the swelling has gone down. Scarless breast reduction provides a significant lift in all patients, though the amount of lift is particularly influenced by the elasticity of the patient’s skin.

Advantages of Scarless Breast Reduction vs. The Older Method

There are many benefits afforded by scarless breast reduction, including:

  • Rapid recovery: While patients are advised to take a full week off from work, many people return to work in just 3 days. 
  • Improved shape of the breasts.
  • No scarring: only two tiny scars per breast at the cannula insertion point.
  • Ability to breastfeed remains in tact.
  • Potential to better detect reading of mammograms.
  • Normal sensation remains in the nipple and areola.
  • Lower cost compared to the traditional method.
  • Excellent option for patients who cannot tolerate a more invasive operation due to medical conditions such as diabetes, hypertension, myasthenia, multiple sclerosis, or asthma.

Scarless breast reduction is also ideal for correcting asymmetrical breasts, a very common condition in which one breast is slightly larger than the other. Most of the time, the inconsistent sizes of a woman’s breasts go unnoticed by others because the breasts are most often fully covered. However, many women are aware of the size difference between their breasts and are distressed by it. Teenage girls with asymmetrical breasts often suffer from severe self-consciousness and want their breasts to be more similar. Scarless breast reduction provides a wonderful option to be able to adjust breast size, without leaving a visible scar.

The Older Method of Breast Reduction

The major disadvantage of the traditional method is that it leaves the patient with 12 inches on scarring on each breast. In the traditional method, two long incisions are made - one along the bottom of the breast, and one that runs from the nipple down the front of the breast in order to remove the fat that is weighing the breast down.  Twelve full inches of scars per breast may be difficult for a young woman to live with. There is risk of bleeding, infection, poor shape, inability to breast feed, and fat, skin or nipple necrosis. 

The older method is generally performed under general anesthesia and can take from two to four hours; blood transfusions are sometimes needed. Because of the extensive cutting used in the older method, some changes in the sensation of the nipple and areola can occur.

The recuperation period can be long, and most surgeons insist that patients do very little activity for several weeks or even months. Patients also report having pain for extended periods after the traditional reduction surgery.

Your Consultation

Your consultation is important for communicating to Dr.Gray how much smaller you would like your breasts to be. Dr.Gray will determine if what you want is realistic for your body and obtainable and he will discuss with you how significant the improvement will be for your body. A consultation will take about one hour from start to finish. Extensive details regarding pre- and post-operative care will be given to you in your care packet at the time of your consultation.

Your Recovery

Follow Dr. Gray’s recommendations carefully and you will have a smooth recovery. You will need to have someone to help you at home for the first 24 hours, often a spouse or a close friend or relative. You will wear a surgical bra for a few days which will keep your breast held in tightly to reduce swelling. It also will feel more comfortable for you that way. Then you will continue to wear a special support bra 24 hours a day except for showering and bathing to continue to keep the swelling down and to ensure that your results are optimal. You will feel more comfortable and secure in it. Although it would be optimal for you to take a full week off from your work or other duties, most people feel very capable of returning to work within three days, although strenuous activity is not recommended for up to one week.  Continuing to not smoke will aid greatly in your recovery. You will be pleased with your smaller breasts right away. Your back and shoulder and neck pain will be reduced right away. Your clothes will fit better immediately and will continue to improve. You will be very happy will the immediate result, which will continue to improve over time. The final results will be seen when the swelling goes down completely, usually in about four to six weeks.

Will I ever need to repeat this surgery? No. You are born with a finite number of fat cells. Fat cells do not grow back. If you gain weight, you will gain it over your whole body, not in one specific area, such as your breasts.
